Use this form method and put the file mailto.cfm in your directory

Here is a sample file(mailto.cfm) just keep adding to it. Edit the mailto.cfm just like a html file.

NOTE Fields in your form, have to be unique. You can not have 2 'dates'. You must label them uniquely.
